Levels of Care

Whether your loved one needs a few hours of companionship each week or more comprehensive daily support, we tailor our care to meet their unique needs, preferences, and goals. As care needs evolve, we adapt alongside them providing families with continuity and peace of mind every step of the way. Our services include companionship and social engagement, assistance with personal care such as bathing, dressing, grooming, mobility, and toileting, dementia and Alzheimer's care, respite for family caregivers, overnight support, and palliative care. We also specialize in hospital-to-home transitions and post-surgical recovery, helping clients recover safely at home while reducing stress for their families. In addition to personal support, we assist with everyday tasks such as meal preparation, light housekeeping, laundry, medication reminders, and accompanying clients to appointments or community activities. Whether care is provided in a private home or within a community, our focus remains the same: helping clients maintain their independence, dignity, and quality of life while giving families confidence that their loved one is receiving compassionate, reliable care.

Care Team

Our care team may include: Personal Support Workers (PSWs) providing assistance with personal care, companionship, mobility support, and activities of daily living. Registered Practical Nurses (RPNs) providing nursing care such as medication administration, wound care, health monitoring, and other clinical services within their scope of practice. Registered Nurses (RNs) providing more complex nursing assessments, clinical oversight, and care coordination when required. Every member of our care team is carefully screened through a comprehensive hiring process that includes interviews, reference and background checks, and competency assessments. We thoughtfully match caregivers based on each client's care needs, personality, preferences, and schedule, with the goal of building trusted relationships and providing continuity of care whenever possible. We provide ongoing supervision, quality assurance, and support. We maintain regular communication with clients, families, and care providers to ensure care remains responsive, consistent, and aligned with each client's evolving needs.
