Care may include and are not limited to help with personal hygiene, dressing, mobility, meal preparation, medication routines, respite, recovery following a hospital stay and support for individuals living with chronic conditions or cognitive changes. Initial Assessment Overview FirstLight Home Care of Halton

Our initial assessment process is designed to ensure every client receives care that is safe, personalized and aligned with their unique needs. We prioritize completing assessments through an in-person visit whenever possible, as this allows us to gather a full picture of the client's environment, routines and overall care needs. Depending on the situation, urgency or client preference, the assessment can also be conducted over the phone or through a video call. During the assessment, a member of our team, often with clinical oversight, will review physical health, cognitive status, mobility, personal care needs and the home environment. We also take time to understand the client's preferences, daily routines, goals of care and any family involvement to ensure the support provided is both effective and meaningful. Following the assessment, we develop a customized care plan outlining the recommended services, schedule and level of support required. This plan is reviewed with the client and their family, with adjustments made as needed before care begins. Once services are in place, we continue to monitor and reassess regularly to ensure the care remains appropriate as needs evolve.
