Our Assessment Process We are committed to making the transition into care as smooth, supportive, and reassuring as possible for both clients and their families. Free Initial Consultation Your journey begins with a complimentary phone consultation, where we take the time to listen, understand your needs, and answer any questions you may have. In-Home Family Consultation When a decision is made to retain LiveWell Pathway, we schedule an in-home consultation. A LiveWell Pathway nurse will visit you and your loved one to conduct a personalized family consultation, gaining a deeper understanding of care needs, preferences, routines, and goals. Customized Care Plan Following the consultation, a detailed and tailored care plan is carefully prepared to ensure the right level of support, comfort, and consistency. Ongoing Follow-Up After the first week of care, we will follow up with a call or visit to check in and ensure everything is going smoothly. We understand there can be an adjustment period, especially for your loved one, and we are here to make any necessary adjustments. Quality Assurance & Oversight For your added peace of mind, a LiveWell Pathway nurse will periodically conduct unannounced visits to ensure your loved one continues to receive the highest standard of care. Periodic Reassessment To ensure care remains aligned with evolving needs, we conduct periodic reassessments of the care plan.
