Qualicare Toronto West provides compassionate, reliable, and personalized home care services designed to help individuals remain safe, comfortable, and independent. We support seniors, adults with changing health needs, and families who need additional assistance for their loved ones. Our goal is to provide consistent, respectful care while giving families peace of mind and helping each client maintain dignity and quality of life.
Levels of Care
We provide a range of care based on each client’s individual needs, including companionship, personal care and assistance with activities of daily living, meal preparation, medication reminders, mobility and fall-prevention support, respite care, dementia and cognitive support, post-hospital and recovery assistance, palliative and end-of-life support, and overnight care. Services can be adjusted as a client’s needs change.
Approach to Care
Our approach is person-centred and family-focused. We recognize that every client has different routines, preferences, abilities, and care needs. We take the time to understand the individual and develop a personalized care plan that promotes safety, independence, dignity, and meaningful daily living. We also value clear communication and collaboration with families and other members of the client’s care team.
Care Team
Our care team may include Personal Support Workers (PSWs), caregivers, Registered Practical Nurses (RPNs), and Registered Nurses (RNs), depending on the client’s needs and required level of care. Caregivers are carefully selected and matched based on the client’s care requirements, personality, preferences, schedule, and compatibility. Our goal is to provide consistent caregivers whenever possible, helping clients feel comfortable and build trusting relationships with their care team.
Certifications, Licenses & Training
Our care team includes qualified and experienced Personal Support Workers (PSWs), Registered Practical Nurses (RPNs), and Registered Nurses (RNs), as appropriate to each client’s care needs. Team members are selected based on their qualifications, experience, and suitability for home care. Training may include dementia and cognitive care, palliative care, infection prevention and control, safe transfers and mobility, fall prevention, medication assistance, and other client-specific care requirements. Regulated nursing professionals maintain appropriate registration and licensing with their respective regulatory colleges.

We take the time to understand the client’s health needs, daily routines, mobility, personal care requirements, home environment, preferences and family support. We maintain ongoing communication and regularly review the care plan so services can be adjusted as the client’s needs change.
