Initial Assessment Overview Nurse Next Door Uxbridge
Our Care Process When a client first reaches out to us, we begin with a brief conversation to understand the type of support they are looking for. This helps us ensure we’re the right fit and allows us to guide them toward the best next steps. We then schedule a Caring Consult, where we take the time to truly understand the client’s goals, preferences, and care needs. During this visit, we also conduct a home assessment to ensure a safe environment for both the client and our caregivers, while offering helpful recommendations to improve comfort and safety. Based on this consultation, we create a personalized care plan tailored specifically to the client’s needs. On the agreed start date, we begin services with a first visit, where we introduce the caregiver to the client and ensure a smooth and comfortable transition. After the first visit, we follow up with the client to gather feedback and make any necessary adjustments to ensure complete satisfaction. To maintain the highest quality of care, we conduct supervisory visits every 60 days, regularly update care plans, and make ongoing changes as the client’s needs evolve.
