About Us
Qualicare North York provides personalized, nurse managed home care that helps individuals live safely and comfortably at home. Services include personal care, companionship, meal preparation, mobility support, medication reminders, and assistance with daily routines. With a focus on the whole person and their family, Qualicare offers compassionate support and professional nursing oversight to promote independence, wellbeing, and peace of mind.
Levels of Care
Qualicare North York supports a wide range of care needs, including assistance with aging at home, dementia care, recovery after surgery or hospitalization, chronic illness, mobility challenges, and palliative and end of life care. We also provide respite care for family caregivers and companionship for individuals who benefit from additional social connection. Each care plan is tailored to the individual, with nurse oversight and practical support to promote comfort, safety, and quality of life.
Approach to Care
What makes Qualicare North York special is our personal, nurse managed approach. With my background as a Registered Nurse and over 18 years of experience, primarily in critical care, I bring a clinical perspective to understanding each client’s needs. We look at the whole picture, including daily routines, safety, wellbeing, and family concerns, and build care around what matters most to them. We also stay involved as needs change, giving families a caring, knowledgeable partner they can turn to.
Care Team
At Qualicare North York, the care team includes nurse leadership, caregivers such as personal support workers, and scheduling and care coordination staff, with the client and family at the centre. We also collaborate with other health professionals involved in the client’s care when needed. Caregivers are selected based on the client’s care needs, the caregiver’s skills and experience, availability, and compatibility with the client’s personality, routines, and preferences. Nurse oversight and ongoing feedback help us maintain a good match and adjust support as needs change.
Accreditation
Qualicare North York is currently working toward accreditation with Accreditation Canada. This process involves reviewing and strengthening our policies, care practices, and quality improvement processes against established standards for safety and quality. It reflects our commitment to accountability, continuous improvement, and providing compassionate, reliable care to our clients and families.
Certifications, Licenses & Training
At Qualicare North York, we verify the qualifications relevant to each team member’s role, including PSW education and current professional registration for nurses. We ensure caregivers receive orientation to each client’s care plan and have the training needed for their assigned responsibilities. Depending on the role and care needs, this may include CPR and first aid, dementia care, infection prevention, safe transfers, and specialized equipment training. Ongoing supervision and education help support safe, consistent care.
Initial Assessment Overview Qualicare North York
Our initial assessment begins with a conversation with the client and family to understand their needs, concerns, and goals. A registered nurse then meets with the client to review their health history, daily routines, mobility, personal care needs, home safety, and existing supports. With the client’s consent, we use this information to develop a personalized care plan, recommend appropriate services, and match caregivers to their needs and preferences. We review the plan as care begins and adjust it as needs change.
