About Us
Home Instead Markham is a locally owned and operated in-home care provider serving families across Markham, including Thornhill and Unionville. We've been part of this community since 2020, and we're owned by the Mitten family — which means decisions are made locally, by people who live in the area. As part of the world's largest home care network, we bring proven systems and training resources to a team that's still small enough to know every client by name. All of our care is nurse-led: experienced nurses build each care plan, train the Care Pros to it, and stay involved as needs change. We're available 24/7 and can often start care within 48 hours — including direct hospital-to-home discharges. Whether a family needs a few hours of companionship each week or around-the-clock live-in support, we build the care around the person.
Levels of Care
We support the full range of in-home care needs, from occasional companionship through to complex, round-the-clock support: - Personal care — bathing, grooming, dressing, mobility, transfers, and washroom assistance - Companionship and social engagement - Alzheimer's and dementia support - Home helper services — meal preparation, light housekeeping, laundry, groceries, and errands - Overnight care - Respite care for family caregivers 24-hour and live-in care - Nurse-directed care — including medication administration, vital signs, glucose monitoring, insulin, oxygen therapy, and wound care - Hospital bedside care - Post-hospital and transitional care - Palliative and end-of-life support - Care management and healthcare navigation Two areas we're especially known for: 24-hour and live-in care, which is one of the most complex arrangements to get right and one of the hardest to staff well, and nurse-directed care, where our Care Pros are trained and supervised by our nursing team to assist with select clinical tasks that most in-home care providers won't take on. We also support clients of all ages, including post-surgical recovery, chronic illness, and disability care.
Approach to Care
When families ask what makes us different, the answer is that we don't just send a caregiver — we manage the whole picture. Every client here is overseen by a nurse, and not just at intake. Our nurses run the initial care consultation, design the care plan, train the Care Pro to that specific client, and stay involved as needs change. That clinical oversight runs through the entire care relationship, which is why our Care Pros can support things like medication administration, oxygen therapy, and diabetic care under nurse direction. We also take matching seriously. Personality, experience, language, interests, daily routine — a good match means better care and far more consistency, which matters enormously to someone who doesn't want a new face every week. Beyond the care itself, we handle coordination: talking to doctors, hospitals, and specialists, managing appointments, and keeping the whole family updated. For adult children managing care from a distance, that's often the part that matters most. And we're built to move quickly. We're available 24/7, and when a discharge planner calls or a family's situation changes overnight, we can often start same-day or next-day. Families rarely get to plan these moments. Underneath all of it, we're part of the world's largest home care network and locally owned by the Mitten family. There's a real people who live in the area that are accountable for every outcome here.
Care Team
A client's care team is built around what they actually need, and it usually spans several roles. Care Pros provide the day-to-day care in the home. Our team includes PSWs alongside experienced caregivers, and we match each client based on skills, schedule, language, and personality fit rather than simply filling an opening. Nurses oversee every client. Our Care Managers are RNs and RPNs who conduct assessments, build the care plan, train Care Pros to each client's specific needs, and carry out ongoing quality visits. This nurse-led model was established by our co-owner Tracy Mitten, RN, whose 30+ years of nursing across chronic disease management and geriatrics shaped how we operate, and it continues to guide the standard our clinical team works to. Our office team handles scheduling, hiring, training, and family communication, and the Markham office is reachable 24/7. Our wider network. For needs outside what we provide directly — physiotherapy, occupational therapy, physicians, and other specialists — we have an established network of trusted professionals we can coordinate with on a client's behalf, so families aren't left to assemble it themselves. We're also part of the Home Instead network, which holds us to operating and training standards that go beyond what's legally required in Ontario.

Home Instead Markham is a licensed Temporary Help Agency under Ontario's Employment Standards Act — a legal requirement to operate a home care company in this province. We're fully insured and bonded, with WSIB coverage for our team, so families aren't taking on liability by bringing us into the home. Every Care Pro we hire clears a police background check before they ever step into a client's home. Our Care Pros complete Home Instead's Person-Centered Care Training for Alzheimer's Disease and Other Dementias, a program recognized by the Alzheimer's Association for its evidence-based approach. Training continues after hire through the Home Instead network, with role-specific education and direct supervision from our nursing team. Where a client receives nurse-directed care, our nurses train and supervise the Care Pro on the specific tasks that client requires.
