About Us
Qualicare Toronto is here to provide compassionate, comprehensive and supportive care to help people of all ages in their homes. Our teams support our clients with personal care, meal preparation, medication reminders, light housekeeping, respite care and companionship.
Levels of Care
We provide care for our clients ranging from 3 hours weekly all the way to 24/7, with a wide range of services. Services include: assistance with daily activities, personal care, post-hospital recovery and respite care.
Approach to Care
Our committed team at Qualicare Toronto will utilize our 360° approach to care to develop a customized care plan that considers four key areas: daily tasks & household management, family & care coordination, companionship and outings, and specialized care (autism, dementia, alzheimer's, acquired brain injuries and more). We work very hard to match our clients with the very best caregivers for this specific needs - taking into account special skills, language, culture and location. Our care is RN led/supervised, providing our caregivers and our clients with an additional level of support. Our customized care plans are monitored on an ongoing basis, and modified as needed, always in consultation with the client and family.

Initial Assessment Overview Qualicare Toronto
We provide an initial assessment by one of our Nurse Care Coordinators, - to design an optimal care plan with inputs from the client and family. We take the time to fully understand the client's needs, health status, the home environment from the perspective of the client and the family members. When the assessment is completed, we propose a care plan that can then be reviewed with the family for input/modification. We work through the matching process to ensure that the caregivers assigned will be the best fit for the client - considering skills, personality, language, culture and client preferences. Care is monitored on an ongoing basis by the RN Care Coordinator, and feedback from the family is always welcome.
