Carevo Home Healthcare can assist with a wide range of care needs, including: Companionship and light homemaking Personal care, including bathing, dressing, toileting and grooming Mobility, transferring and fall-prevention support Meal preparation and medication reminders Respite care for family caregivers Dementia and Alzheimer’s care Post-hospital and post-operative recovery support Nursing care, including medication administration, wound care and health monitoring Palliative and end-of-life care Foot care, physiotherapy and occupational therapy Continuous or 24-hour care for individuals with complex needs Care can be tailored from occasional short visits to daily, overnight or around-the-clock support, depending on the client’s needs.

A consistent core group of caregivers—not constantly changing PSWs. Direct access to a named nurse or Care Manager. Fast local response and scheduling across Durham through Bay of Quinte. One team supporting the client from assessment and/or hospital discharge through ongoing home care.
Care Team
We have Personal Support Workers(PSW's), Registered Practical Nurses(RPN's), Registered Nurses(RN's), a Nurse Practitioner(NP), Physiotherapists(PT's), Footcare Nurses and hairdressers. When matching clients with their caregivers we take into consideration their preferences, care plan and personalities.

Initial Assessment Overview Carevo Home Healthcare
Carevo’s assessment process begins with a free consultation and generally follows these steps: Initial conversation: A care coordinator speaks with the client or family about the situation, location, requested services and preferred schedule. Care assessment: A nurse coordinator reviews the client’s: Medical conditions and current health concerns Bathing, dressing, toileting and grooming abilities Mobility, transferring and fall risks Eating, meal preparation and medication needs Memory, dementia-related behaviours and supervision needs Daily routines, personal preferences and home-safety risks Existing family, Ontario Health atHome and insurance supports Personalized care plan: Carevo documents the assistance required, recommended caregiver qualifications, visit frequency, schedule and any nursing instructions. Caregiver matching: The client is matched with a caregiver based on experience, skills, personality and availability. Ongoing review: The nurse coordinator monitors the care plan and adjusts it if the client’s health, mobility, routines or support needs change
