Initial Assessment Overview Home Instead Durham East

Home Instead Durham Region begins with a free, no-obligation Care Consultation designed to help families better understand their loved one’s needs, routines, preferences, and goals. During this initial assessment, our Care Coordinators asks thoughtful questions to learn where support may be helpful, while also identifying what the individual is still managing well on their own. The assessment may include a review of Activities of Daily Living, such as bathing, dressing, meal preparation, mobility, transferring, and other day-to-day tasks. We also look at home safety, social connection, health concerns, medication reminders, and any areas where additional support could improve comfort, independence, and peace of mind. After the consultation, we review the information with the family and make care recommendations based on the individual’s needs. Together, we discuss the types of care, schedule, and level of support that may be the best fit, helping families make informed decisions with confidence.
