Approach to Care

We believe staying at home should mean staying engaged. Every service is delivered by trained caregivers and supervised by a registered Nurse Manager, and every care plan is measured against the national accreditation standard we are held to. Care can be in place within 24 to 48 hours. Plans are client-centered and set collaboratively with the family, physician, and support network, and they keep clients physically, mentally, socially, and emotionally active to help preserve cognitive function and independence. Care follows the client across settings, at home, during a hospital stay, and through any transition or placement, adjusting as needs change. Full transparency for families is available through the Online Family Portal where daily shift notes, nurse summary notes, care plans, schedules and resources are posted.

Care Team

Registered Nurse Managers: supervises every care plan and oversees quality of care; in good standing with College of Nurses of Ontario Registered Nurses: RNs and RPNs in good standing with College of Nurses of Ontario Personal Support Workers (PSWs): hands-on daily care and companionship, certified in GPA® and U-First!® Our company is working towards having all PSWs registered with the Health Care Services Provider Oversight Authority from the Province of Ontario Medical Director (Family Physician): clinical guidance with palliative and geriatric expertise

Accreditation

HosPall holds Accreditation Canada's Exemplary Standing, the highest national standard in Canadian home care. Exemplary Standing is awarded to organizations that go beyond the requirements of the accreditation program and demonstrate excellence in quality improvement. Every care plan we deliver is measured against that standard and supervised by a registered Nurse Manager, so families get third-party proof that our practices, staff, and processes have been independently reviewed and verified, not just claimed. Care at home should be held to the same standard as care anywhere in the health system. Accreditation is our commitment that it is: verified quality, real accountability, and peace of mind for your family.

Initial Assessment Overview HosPall Homecare

When You First Call HosPall The first call is exploratory, with no pressure and no obligation. Our aim is simply to answer your questions and help you understand your options. Your call is answered by an experienced nurse — not a call centre or intake clerk — so the very first conversation is informed, knowledgeable, and reassuring. Everything you share is fully confidential. Many families tell us they delayed reaching out because they weren't sure what information they needed or what to expect. You don't need to have anything prepared. Just call. Scheduling the Initial Assessment If you'd like to take the next step, we schedule an in-person assessment within 12 to 24 hours, conducted by an experienced Registered Nurse. The assessment can take place wherever the client is — at home, in hospital, or in a care facility. If the client is currently in hospital, we can coordinate directly with the discharge team to ensure a smooth transition home. There is no cost for the assessment. During the Assessment Visit Our Registered Nurse takes the time to: Meet with the client and family together Answer your questions about home care and what to expect Discuss care options, requirements, and preferences Talk through possible timing and start dates Service Agreement and Care Start Within 4 hours of the assessment visit, if the client has agreed to move forward, we send a service agreement. Start dates and schedules are finalized once it has been reviewed. Once the agreement is signed, services can typically begin within 12 to 24 hours, with a dedicated Registered Nurse Manager assigned to oversee the client's care. The full assessment and ongoing regular Nurse Manager visits are included in our hourly care rate — there are no separate assessment or supervision fees. Your Care Plan and Family Portal A full, individualized care plan is developed in consultation with the family. Each family receives access to a secure online family portal with full visibility into: The full care plan The schedule of visits Assigned caregivers and staffing Care notes after each shift So you always know exactly what is happening and who is providing care. Our Two-Week Trust Period We know that starting home care is a significant decision. Within the first two weeks of services beginning, families can cancel at any time, with no notice required. We believe trust is earned, not contracted.
