Initial Assessment Overview Qualicare Durham Region
At Qualicare Durham Region, we take a thoughtful and personalized approach to getting started. Every new client begins with a comprehensive in-home assessment conducted by one of our nurse Care Managers. During this visit, we take the time to understand the client’s health status, daily routines, preferences, home environment, and overall goals for care. We also listen closely to family members to ensure their concerns and priorities are fully understood. Following the assessment, we develop a customized care plan tailored to the client’s unique needs. This plan outlines recommended services, schedules, and any specific clinical considerations. We carefully match each client with caregivers who are not only qualified, but also a good personality fit. Once care begins, our nurse Care Managers provide ongoing oversight, regular check-ins, and adjustments to the care plan as needs evolve—ensuring consistent, high-quality support every step of the way.
