Qualicare Durham Region provides compassionate, personalized home care that helps people of all ages remain safe, independent and comfortable at home. Our experienced caregivers offer personal care, companionship, meal preparation, housekeeping, medication assistance, respite care and support for individuals living with dementia or complex health needs. Having successfully achieved accreditation twice, we are committed to maintaining high standards of quality, safety and client-centred care. With professional care coordination and 24/7 support, we create flexible care plans tailored to each family’s unique needs.
Levels of Care
We support a wide range of care needs, from companionship and help with daily activities to personal care, respite care, post-hospital recovery and assistance for individuals living with chronic illness or disability. We specialize in dementia and memory care, with enhanced training provided to all our Personal Support Workers, as well as compassionate palliative and end-of-life care for individuals who wish to remain at home. We can also support clients with complex health needs through professionally coordinated, personalized care plans that adapt as their needs change. Some of the services we provide include: Personal care (bathing, dressing, mobility support) Companionship and social support Dementia and Alzheimer’s care Palliative and end-of-life care at home Respite care for family caregivers Meal preparation and light housekeeping Medication reminders and support Nursing oversight and care coordination Post-hospital and recovery care 24-hour and live-in care options
Approach to Care
What makes Qualicare Durham Region special is that we combine the warmth and personal attention of a locally owned company with the professional standards and resources of an established home-care organization. Our care is nurse-led, thoughtfully coordinated and tailored to the whole person—not simply a checklist of tasks. We take time to understand each client’s needs, routines and preferences, carefully match them with the right caregivers, and remain available to families 24/7. With specialized expertise in dementia and palliative care and two successful accreditations, families can feel confident that their loved one is receiving compassionate, dependable and high-quality care.
Care Team
Depending on the client’s needs, the care team may include Personal Support Workers, Registered Practical Nurses, Registered Nurses and a dedicated Care Coordinator. We also collaborate with the client’s physicians, therapists and other community healthcare professionals to support a coordinated approach to care. Caregivers are selected based on the client’s required level of care, the caregiver’s training and experience, availability and location, as well as personality, communication style, interests, language and cultural preferences. We place great importance on compatibility and continuity—not simply filling a shift. The care relationship is monitored by our coordination team, and we welcome feedback so adjustments can be made whenever needed.
Accreditation
Qualicare Durham Region has earned accreditation twice through CARF International, an independent, internationally recognized accrediting organization for health and human services. Achieving CARF accreditation requires a rigorous review of our operations, policies and care practices, including client safety, service quality, staff training, care planning and continuous improvement. For clients and families, it provides added assurance that our organization has been independently evaluated against recognized standards and is committed to delivering safe, accountable and high-quality care.
Certifications, Licenses & Training
Our care team includes formally trained Personal Support Workers and nurses registered with the College of Nurses of Ontario. All caregivers complete CPR and First Aid, infection prevention and control, medication assistance, and safe transfer and equipment training, along with education tailored to their roles and client assignments. We place a strong emphasis on ongoing professional development. Our caregivers receive enhanced training in dementia and memory care, including continued education through the Alzheimer Society of Durham Region’s Dementia Friendly Communities program. We also provide specialized palliative and end-of-life care training developed and delivered by our Executive Director, a Palliative Care Resource Nurse with 30 years of extensive frontline home-care experience. At Qualicare Durham Region, we take a thoughtful and personalized approach to getting started. Every new client begins with a comprehensive in-home assessment conducted by one of our nurse Care Managers. During this visit, we take the time to understand the client’s health status, daily routines, preferences, home environment, and overall goals for care. We also listen closely to family members to ensure their concerns and priorities are fully understood. Following the assessment, we develop a customized care plan tailored to the client’s unique needs. This plan outlines recommended services, schedules, and any specific clinical considerations. We carefully match each client with caregivers who are not only qualified, but also a good personality fit. Once care begins, our nurse Care Managers provide ongoing oversight, regular check-ins, and adjustments to the care plan as needs evolve—ensuring consistent, high-quality support every step of the way.
