Integracare’s intake process begins with an initial inquiry where the team gathers a high-level understanding of the client’s needs, followed by a complimentary nurse-led assessment conducted in-home, in hospital, or virtually. From there, a personalized care plan is developed, and a dedicated Nurse Care Manager is assigned upon confirmation of services. Caregivers are then carefully screened and thoughtfully matched based on both clinical needs and personality fit. Once scheduling, rates, and logistics are finalized, care can begin within days—sometimes sooner if urgent—with 24/7 support available. Ongoing nurse-led oversight, regular communication, and continuous reassessment ensure care remains consistent, responsive, and aligned with the family’s needs.
