Levels of Care

We offer three levels of care depending on where someone is in their aging journey. Companion & Independence Support — Regular visits focused on companionship, light household help, meal support, and staying socially connected. Ideal for seniors who are managing well but benefit from consistent company and a watchful eye. Daily Living Support — Hands-on personal care including bathing, dressing, mobility assistance, and medication reminders, delivered by trained PSWs with a consistent schedule and nurse oversight. Complex & Changing Needs — Intensive support for clients with dementia, Parkinson's, chronic conditions, or recovering from hospitalization. RN-supervised care plans, adjusted as needs evolve. All levels include nurse-led assessment and the same caregiver every visit.

We take the time to understand the person, not just the diagnosis. From there, we build a care plan that reflects their routines, preferences, and what a good day actually looks like for them. We assign the same caregivers to every visit. We maintain clinical oversight at every level of care. And when something changes — and it always does — we adjust before it becomes a crisis. Over 12 years in Burlington, Oakville, and Dundas, we've learned that families don't just need good care. They need to trust that someone is paying attention. That's what we're here for.

Care Team

Every Qualicare Burlington client has a care team built around their specific situation. At the centre is a Personal Support Worker — a trained, consistent caregiver who handles day-to-day personal care, companionship, and household support. All care is clinically overseen by our Nurse Manager, a Registered Nurse who conducts the initial in-home assessment, writes the care plan, and remains the point of contact for any clinical changes. For clients with more complex needs, nursing visits are built directly into the care schedule. We do not rotate caregivers. When we match a PSW to a client, we consider the person's personality, routines, care needs, and preferences — not just availability. That match is made before the first visit, and we stand behind it. If something isn't working, we fix it quickly. For clients requiring specialized support — dementia care, Parkinson's, post-hospitalization recovery — we assign caregivers with direct experience in those areas

Every new client begins with a no-charge in-home consultation conducted by our Registered Nurse. This is not a standard intake call — it is a clinical assessment that looks at the full picture: physical functioning, cognitive and emotional wellbeing, home safety, caregiver strain on the family, and the social factors that affect how someone is actually managing day to day. From that assessment, our RN develops a personalized care plan — one that reflects not just what the client needs, but what matters most to them. The plan covers care tasks, caregiver matching criteria, scheduling, and the clinical indicators we will monitor over time. The family is part of that conversation from the start. Before any care begins, they understand exactly what the plan includes, who will be coming into the home, and how to reach us if anything changes. Care does not start until the plan is in place and the right caregiver has been identified.
